1. Acceptance of Terms
By scheduling an appointment, attending sessions, or using any services provided by Mad World Wellness ("we," "us," "our"), you ("client," "you," "your") agree to be bound by these Terms & Conditions. These terms constitute a legally binding agreement between you and Mad World Wellness.
2. About Our Services
Mad World Wellness is a clinical psychotherapy private practice operated by two licensed clinical social workers who are equal partners and owners. We provide individual therapy services to clients ages 8-90 in a supportive, confidential environment.

2.1 Scope of Services

Referral services when appropriate

2.2 Location of Services
​
Our practice operates from our private residence, which serves as both our business location and personal home. By accepting these terms, you acknowledge and respect the residential nature of our office space.
3. Client Rights and Responsibilities
3.1 Your Rights

Right to confidential treatment in accordance with HIPAA and Michigan state law

Right to informed consent regarding treatment

Right to access your health records as permitted by law

Right to file complaints with appropriate regulatory bodies

Right to terminate services at any time
3.2 Your Responsibilities

Arrive on time for scheduled appointments

Provide accurate and complete information about your health and circumstances

Participate actively in your treatment

Pay fees as agreed upon

Respect our office policies and residential setting

Notify us immediately of any changes to contact information or insurance
4. Scheduling and Attendance
​
4.1 Appointments
​
Appointments are scheduled in advance and confirmed via phone, email, or our secure client portal. We recommend scheduling regular appointments to maintain therapeutic continuity.

4.2 Cancellation Policy

24-Hour Notice Required: Appointments must be cancelled at least 24 hours in advance

Late Cancellations: Cancellations with less than 24 hours notice may be charged the $75 no-show fee

No-Shows: Clients who miss appointments without notice will be charged a $75 no-show fee

Emergency Exceptions: We understand that true emergencies occur and will work with you on a case-by-case basis

4.3 Late Arrivals

Sessions will end at the scheduled time regardless of late arrival. You will be responsible for the full session fee.

5. Payment Terms and Policies

Session fees are due at the time of service unless other arrangements have been made

We accept cash, check, and credit/debit cards

Payment plans may be available upon request and approval
5.2 Insurance

We work with select insurance providers

You are responsible for understanding your insurance benefits and coverage limitations

Co-pays and deductibles are due at time of service

We will file claims on your behalf as a courtesy, but you remain ultimately responsible for all charges
5.3 Outstanding Balances

Accounts more than 60 days past due may result in suspension of services

Collection activities may be initiated for unpaid balances

A reasonable collection fee may be added to outstanding accounts
6. Confidentiality and Privacy

6.1 Protected Health Information
​
We are committed to protecting your privacy in accordance with HIPAA and Michigan state law. Your health information will only be disclosed as permitted by law or with your written authorization.

6.2 Mandatory Reporting
​
We are required by law to report:

Suspected child, elder, or vulnerable adult abuse

Imminent danger to self or others

Court-ordered disclosures

Other situations as required by Michigan law
6.3 Recording Policy

Sessions may not be recorded by clients or any third parties. Our office is located in our private residence, and we do not consent to any audio, video, or digital recording of our sessions or premises.

7. Electronic Health Records
​
We use SimplePractice, a HIPAA-compliant electronic health record system, to maintain your treatment records. This system helps ensure the security and accessibility of your health information while maintaining appropriate privacy protections.

8. Emergency Procedures
​
8.1 Crisis Situations
​
Mad World Wellness is not an emergency service. In case of a mental health emergency:

Immediate Danger: Call 911

Crisis Support: Contact the National Suicide Prevention Lifeline at 988

Local Crisis Services: Contact your local emergency mental health services
8.2 After-Hours Contact

We do not provide 24-hour crisis services. For urgent but non-emergency situations between sessions, you may leave a voicemail, and we will return your call within one business day.
9. Limitations and Disclaimers
​
9.1 No Guarantee of Results
​
While we are committed to providing quality care, we cannot guarantee specific outcomes or results from therapy. Mental health treatment is a collaborative process that requires active participation from all parties.
9.2 Scope of Practice
​
Our services are limited to psychotherapy and counseling within our scope of practice as licensed clinical social workers. We do not prescribe medication or provide medical services.
9.3 Limitation of Liability
​
Our liability is limited to the amount paid for services rendered. We are not liable for indirect, consequential, or punitive damages.

10. Termination of Services
​
10.1 Client-Initiated Termination
​
You may terminate services at any time. We encourage you to discuss your decision during a session to ensure proper closure and provide appropriate referrals if needed.
10.2 Therapist-Initiated Termination

We may terminate the therapeutic relationship if:

Treatment goals have been achieved

We determine we cannot provide effective treatment

There are safety concerns

Payment obligations are not met

Terms and conditions are violated
10.3 Termination Process

When services are terminated, we will provide appropriate referrals and transfer records as requested and permitted by law.

11. Technology and Communication
11.1 Electronic Communications

We may communicate via secure email and text messaging platforms

Standard email and text messaging are not secure and should not be used for sensitive information

You may opt out of electronic communications at any time
11.2 Telehealth Services

When applicable, telehealth services are subject to additional terms and technological requirements that will be provided separately.

12. Changes to Terms

We reserve the right to modify these Terms & Conditions at any time. Changes will be communicated in advance, and continued use of our services constitutes acceptance of any modifications.
13. Governing Law

These Terms & Conditions are governed by the laws of the State of Michigan. Any disputes will be resolved in the courts of Michigan.
14. Severability

If any provision of these Terms & Conditions is found to be unenforceable, the remaining provisions will continue in full force and effect.
